Care 4 Caregivers is designed to change that. The program helps you feel more confident in your caregiving role, access support sooner, care for your own well-being alongside your loved one, and take small, manageable steps toward better outcomes.

With the program, you can:

  • Learn practical skills to handle everyday caregiving challenges
  • Recognize when to seek help and where to turn
  • Connect to services like respite care, Medicare support, and mental health resources
  • Stay supported without feeling overwhelmed

Care 4 Caregivers combines 3 key tools to support you on your caregiving journey:

Caregiver Counseling: Personalized support designed for your unique needs
Because every caregiving situation is unique, Care 4 Caregivers connects family caregivers with a trained and experienced case manager who can provide individualized caregiver counseling and support based on the caregiver’s needs and circumstances. Caregiver counseling is a Family Caregiver Support Program (FCSP) service designed to help caregivers manage the emotional, physical, and practical challenges associated with caring for a loved one. 

Caregiver counseling may include: 

  • Individual counseling sessions by phone or in person (appointment only)
  • Support for caregiver stress, depression, grief, and loss related to caregiving responsibilities
  • Problem solving and coping strategies for challenging caregiving situations
  • Involvement of family members and informal support systems when appropriate
  • Connections to respite care, support groups, and other community resources
  • Ongoing guidance, encouragement, and follow-up throughout the caregiving journey

The goal of caregiver counseling is to strengthen the well-being, reduce caregiver burden, and help caregivers feel more confident and supported in their caregiving role.

Care Communications: Encouragement and support delivered right to your phone
After enrolling, you’ll receive thoughtful text messages designed to guide and encourage you throughout your caregiving journey. These messages may include:

  • Helpful tips and reminders
  • Links to short trainings and educational resources
  • Interactive check-ins or self-guided tools
  • Information on topics like mental health, Medicare, and respite care
  • Encouragement and reminders to prioritize your own well-being 

Care 4 Caregivers Library: Your curated caregiver learning hub
Care 4 Caregivers is an interactive digital library filled with trusted, caregiver-focused resources.

Inside the Care 4 Caregivers library, you’ll find:

  • Short videos and guided learning on caregiving topics
  • Personalized content based on your interests and needs
  • Wellness resources to support stress management and self-care
  • Information on caregiving, aging, dementia, community resources, and long-term planning
  • Easy-to-follow sections that help you learn at your own pace

Care 4 Caregivers (C4C) is for anyone providing unpaid care or support to a family member, partner, friend, or loved one. C4C provides support, resources, and optional one-on-one guidance to help you manage your challenges, reduce stress, and feel more confident in your caregiving journey.
