Senior Connections is an in-person case management program that connects older adults and dependent adults who have been negatively impacted and isolated by the COVID-19 pandemic to services that may enhance their general quality of life including social, recreational and informational activities.

Eligibility:
Residents of Sacramento County
Older adult (60 years old and older) -OR -
Dependent adult (ages 18-59) who are unable to meet their own needs and are victims of abuse, neglect, or exploitation.

Agency on Aging Area 4 is fully committed to providing accessible facilities, elements and
channels of communication to all members of the public. As part of this commitment, Agency on Aging Area 4
